‌1. Fully Filled-Up and Signed Application Form
2. Valid Passport (at least six (6) months valid
3. Two (2) pcs of recent photo w/white background (35mm x 45mm)
4. Copy of all issued visas, pages of passport with stamps (travel history)
5. Cover Letter with purpose of travel and Day-to-Day Itinerary
6. Proof of Hotel/Accommodation
7. Roundtrip Flight Reservation
8. For Family Visit, proof of relationship with the host (Marriage Contract, Birth Certificate etc)
9.Proof of Employment:
If employed: Certificate of employment w/ position, salary and tenure
Approved leave of absence
Latest Income Tax Return (ITR)
If business owner: Complete Business Registration (Current Business License, DTI/SEC and BIR Registration)
Latest Company Income Tax Return (ITR)
10. Evidence of Sufficient Funds :
Bank Certificate & Statement of Account
Bank Book
11. Proof of Assets: Car Registration, Land Titles etc.
12. For students/minors:
Proof of financial capacity of parents/legal guardian
Copy of passport of parents/legal guardian
Affidavit of Support and Consent
School Certificate of Enrollment with ID
Approved Leave of Absence if travelling during the school year
DSWD Clearance if not travelling with either parent
13. NSO Marriage Contract (if married)
14. NSO Birth Certificate
15. For Non-Philippine Passport Holder, Copy of Alien Certificate of Registration (ACR)
16. International Travel Insurance (Minimum coverage of EUR30,000.00)
17. Other requirements may be required by the consul/embassy if necessary
