PASSPORT ASSISTANCE

New Applicants:
- Duly accomplished Application Form & Undertaking form
- Two (2) Valid Government issued IDs (if not available need supporting documents)
- NSO Birth certificate (If Married for female - NSO Marriage Contract)
- Supporting Documents : NBI,Transcript of Records, Form-137,
  Baptismal Certificate, Year Book w/ Pic (indicating Birthdate & Place) ~ Old IDs
For Minor:
- NSO Birth certificate, Passport of Parents(/Mother)
- If traveling w/ Guardian
  (Notarized Affidavit of Support &
   Consent of either Parent indicating relationship of the Guardian)
  (Original & Photocopy of DSWD Clearance)
Renewal:
- Duly accomplished Application Form & Undertaking form (Original)
- Old Passport
Personal Appearance(for Picture taking, Signature and Verification):
- Scheduled within 3 to 5 working days
Release of Passport:
- more or less 10 working days from the date of Appearance
Processing Fee:
- Php2,200 per applicant
