Original Valid Passport (at least 6 months valid)
Old and Photocopy of All Passport and Issued Passport and Immigrations Stamps (Detailed Travel History)
Two (2) Recent Passport Size Color Photo
Proof of Employment:
If employed: -Certificate of employment w/ position, salary and tenure
-Approved leave of absence
-Latest Income Tax Return (ITR) 2316
If business owner: -Complete Business Registration (Current Business License, DTI/SEC and BIR Registration)
-Latest Company Income Tax Return (ITR) -Original OR Certified True Copy
Evidence of Sufficient Funds: Bank Certificate and Certified True Copy of Bank Statement /Time Deposit Certificate
Proof of Assets (Certified True Copy): Car Registration, Property, Land Titles etc.
For Minors/Student: -Proof of financial capacity of parents/legal guardian
-Affidavit of Support and Consent
-School Certificate with valid school ID
-If not travelling with either parent, DSWD Clearance
NSO Marriage Contract (if applicable)
NSO Birth Certificate
Proof of Accommodation and Travel Details:
-Flight Booking
-Hotel Voucher/Booking
-Day-to-Day Itinerary
